For your convenience, we accept Visa and MasterCard. We offer the Cosmetic Fee Plan for services over $1500. We strive to deliver the finest care at a reasonable cost to our patients. In order to keep costs down, payment is due at the time service is rendered. If you have questions regarding your account, please contact us at 480-657-7065 and ask for the billing department. Many times, a simple telephone call will clear up misunderstandings.
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage or payment.
We will send you a monthly statement. Most insurance companies will respond within 4 to 6 weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. If your insurance pays more than anticipated, you will be sent a refund check in the mail. Your prompt remittance is appreciated.
