First Visit

Your initial appointment will consist of a consultation to evaluate your concerns, explain your diagnosis and discuss treatment options. Occasionally, minor surgical procedures can be done the same day as the consultation. However, a complex medical history or treatment plan will require an initial evaluation and a later appointment to provide treatment.


Please assist us by bringing the following information at the time of your initial visit:

  • The surgical referral slip from your dentist or doctor and any X-rays if applicable.
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring your insurance card and information. This will save time and allow us to help you process claim.
  • Please fill out your health questionnaire under Patient Registration and send it to us via e-mail at karen@guyettesurgery.com. This will save you time when you arrive for your initial visit.


IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or legal guardian at the consultation visit. Without parental consent, we cannot evaluate and treat minors.

A pre-operative consultation and examination is mandatory for patients undergoing IV anesthesia for surgery. If you are having IV sedation, do not have anything to eat or drink 6 hours prior to your surgery. You will also need an adult to drive you home.

Whether online or in the office, please take the time to accurately fill out your health questionnaire. It will allow us to better understand your health issues and prevent problems.

X-Ray

If your dentist or physician has taken x-rays within the past 6 months, please request that the films be forwarded to our office. If there is not enough time, please pick up the x-ray films and bring them to our office for your initial visit. If additional views are necessary, they can be taken at our facility.
