Patient Portal and Registration

It is extremely helpful to complete your patient health history before you come into the office at Guyette Facial & Oral Surgery Center. To assist with this, you can simply open the link for our Patient Registration Form, and you’ll be able to fill that out from your computer or phone and submit. This will send the encrypted document to our office under a secure server, and we will create (or update) your chart. Be sure to look for the check mark on your computer verifying the submission was successful. You can also complete your insurance information on this form.

When we receive this information from you, we will be able to contact your insurance company to find out what benefits you have available for the procedure(s) you desire. You will also have the choice to print your completed form as well.
